Care delivery within community mental health teams by the HSIB

The Healthcare Safety Investigation Branch (HSIB) published an independent report on care delivery in community mental health teams (CMHT) in March 2023 that is designed for healthcare organisations, policymakers and the public, with the goal of helping to improve patient safety. The report uses as an example, a real patient safety incident in which a woman died by suicide while under the care of a CMHT, which has been used to examine national issues.
The following four areas were looked at:
- assessing a patient’s risk of self-harm or suicide
- considering menopause as a risk factor for mental health conditions
- engaging with families
- caring for people with a first episode of psychosis
