Our clinical pharmacists, supported by palliative care specialists, receive training, supervision, and ongoing support to work effectively with hospices
They first complete the European Certificate in Essential Palliative Care to enhance their skills, with other courses and attendance at conferences planned to keep their palliative care knowledge up-to-date. Our Lead Palliative Care Pharmacist Karen Higlett, who has over 15 years experience working in palliative care, is there to ensure an outstanding service for all our hospices.
Our pharmacists provide medication advice and clinical review of prescriptions to ensure medication is used safely and appropriately. They carry out medicine management audits to ensure medication is being handled in accordance with best practice and in compliance with regulatory requirements. We can also provide governance support including higher level ‘Chief Pharmacist’ services, such as comprehensive policy and PGD review, to ensure best practice and professional standards in medicines management are maintained.

Our Services
Enhancing the quality of medicines management on-site; we ensure safety and maintain high standards while identifying opportunities for improvement through clinical screening of all prescriptions, review of medicines related incidents and involvement with the wider MDT and medicines management meetings. All of this promotes safe and effective prescribing and supports a culture of continuous improvement.
We will support your medicines reconciliation processes ensuring that patients are maintained on their current medication, unless appropriate deprescribing is indicated, enabling seamless transfer between settings.
Supporting the development of Policies and Procedures with input from our clinical pharmacist, ensuring they are in line and are up to date with regulatory and best practice guidelines.
Regular audits and support with medicines management to ensure compliance with regulations and best practice.
Providing training to ensure your staff are competent, and understand medicinesmanagement as appropriate.
